Questions and Answers
Breast Enlargement FAQs

How do you figure out the size of the implant to use?
We use a combination of measurements – basically we try to match the width of the implant to the width of your natural breast, and then go up or down in size from there. We find that trying on implant sizers in a sports bra is also helpful. Remember, the ideal size is different for each person – and you are “in the driver’s seat” when it comes to picking the size.

What kind of anesthesia do you use? How long does the surgery take?
We use general anesthesia, given by an MD Anesthesiologist. The surgery takes about an hour and a quarter.

What kinds of incisions do you use?
The most popular ones are the “infra-mammary” (in the crease under the breast) and the “peri-areolar” (around the lower part of the nipple). We usually let most patients have a choice between the two.

Do you put the implant over or under the muscle?
Usually under the muscle – there are numerous reports showing this to be advantageous for the patient, with better quality mammograms, more natural shape, and less risk of developing firmness around the implant. The “dual-plane” approach is also popular. If there are certain reasons why you would like the implant to be “over” the muscle, we can also use that method.

Which implants do you work with?
We usually use the round, smooth implants, rather than textured teardrop-shaped ones. Most of my patients want a little more fullness in the upper part of the breast – and the round implants work well to achieve this. Both Inamed (now part of Allergan) and Mentor make good quality implants.

How long should I take off from work?
Most patients take 5-7 days off from work, and about 3 weeks off from vigorous exercise or working-out.

“Dr. Fiala’s credentials were superior to the other doctors. The staff made me feel very welcome and comfortable. I was told that Dr. Fiala was the best cosmetic surgeon in the area by my referral source.” -VM
