Breast implant removal is a significant decision that women consider for various reasons. Some choose to remove their implants due to discomfort, lifestyle changes, health concerns, or dissatisfaction with their appearance. No matter the reason, it’s essential to understand what to expect before, during, and after the procedure. Proper preparation and knowing the recovery timeline can help ensure a smooth process and the best possible results.

Consultation and Deciding on Surgical Options

The first step in your journey is a consultation with a board-certified plastic surgeon. This appointment is crucial because it allows you to discuss your concerns, medical history, and desired outcomes. Your plastic surgeon will examine your breasts, assess the condition of the implants, and recommend the best approach for removal.

Dr. Thomas Fiala offers two main surgical options for breast implant removal:

  • Simple Removal (Explantation Only): This procedure involves removing the breast implants without removing the surrounding scar tissue, also known as the capsule. It is a straightforward process with a shorter recovery time and is best suited for patients who do not have complications such as capsular contracture or implant rupture.
  • Explantation with Total Capsulectomy: This method involves removing both the implants and the surrounding scar tissue. It is often necessary for patients experiencing complications such as hardened scar tissue (capsular contracture) or silicone implant rupture. It is optional for those experiencing symptoms of breast implant illness if the implants are intact.

As a plastic surgeon with over 25 years of experience, Dr. Fiala will help you decide which option is best for your situation, taking into account your medical needs, aesthetic goals, and any additional procedures, like a breast lift or fat grafting to the breast.

Preparing for Breast Implant Removal Surgery

Once you have decided to move forward with breast implant removal, our nurses will provide you with detailed preoperative instructions. Proper preparation can help ensure a successful surgery and smooth recovery. Here are a few key steps to follow:

  • Medical Evaluations: You may need blood tests to assess your overall health or an ultrasound scan of the breast to check the condition of your implants.
  • Medication Adjustments: Some medications, such as blood thinners, will need to be stopped before surgery to reduce the risk of excessive bleeding.
  • Lifestyle Changes: Avoid smoking and limit alcohol intake before surgery, as these can impact healing and increase the risk of complications.
  • Arrange Transportation and Assistance: Since breast implant removal is performed under anesthesia, you will need someone to drive you home after the procedure. It’s also helpful to have a friend or family member assist you for the first few days of recovery.
  • Prepare Your Recovery Area: Stock up on essentials such as comfortable clothing, extra pillows for support, and any necessary medications before surgery.

By taking these steps, you can help ensure a smooth and stress-free recovery process.

What Happens During Explant Surgery?

Breast implant removal surgery is typically performed under general anesthesia and can take one to four hours, depending on the complexity of the procedure and whether capsulectomy and/or mastopexy are performed.

Here’s what you can expect during the procedure:

  1. Nerve Block: After you are asleep, Dr. Fiala will place an ultrasound-guided nerve block to numb the breast and help keep you comfortable.
  2. Incisions: Dr. Fiala will make an incision, usually along the same location as your original breast augmentation surgery, to minimize additional scarring.
  3. Implant Removal: The implants are carefully removed from the breast pocket.
  4. Capsule Removal (If Needed): If a total capsulectomy is performed, the scar tissue surrounding the implants will also be removed.
  5. Drain Placement (If Needed): When a total capsulectomy is performed, small surgical drains are placed to help prevent fluid buildup and promote healing.
  6. Closure and Dressing: The incisions are closed with sutures, and a supportive bandage or surgical bra is applied to help with healing.

Recovery After Breast Implant Removal

The recovery process varies depending on the individual and the type of procedure performed. Below is a general timeline of what you can expect:

First Few Days After Surgery

  • Swelling, bruising, and mild discomfort are normal.
  • Pain medication may be prescribed to help with any soreness.
  • You will need to rest and avoid any strenuous activities.
  • If surgical drains are placed, your surgeon will provide care instructions.
  • You should wear a supportive surgical bra to minimize swelling and support healing.

One to Two Weeks Post-Surgery

  • Most patients can return to light daily activities.
  • Swelling and bruising will start to subside.
  • Strenuous exercise and heavy lifting should still be avoided.
  • Your surgeon may remove sutures and drains if they were placed.

One to Three Months Post-Surgery

  • Most swelling should resolve, and your breasts will start to settle into their natural shape.
  • If you experience any unusual pain, lumps, or changes in your breasts, you should contact your surgeon immediately.
  • Scars will begin to fade but may take several months to a year to fully diminish.
