Spending on Cosmetic Procedures Exceeds $12 Billion in the US in 2014: ASAPS Report

Aesthetic Procedures | Surgical | Non Surgical | Altamonte Springs FLConstant innovation in the field of cosmetic surgery and non-surgical aesthetic procedures has helped make these procedures more predictable, effective, and affordable. Each year millions of new American patients as well as repeat patients continue to seek treatments to enhance their physical appearance and reduce the signs of aging.

Board certified plastic surgeon, Dr. Thomas Fiala, provides stellar, acute, and fascinating cosmetic procedures. Dr. Fiala believes in adopting a holistic approach to every cosmetic treatment, and focuses on delivering results in the least invasive and a healthy manner. He receives patients from Orlando, Altamonte Springs, FL for a wide range of aesthetic surgery and non-surgical procedures.

Increased Acceptance of Aesthetic Procedures

The American Society for Aesthetic Plastic Surgery (ASAPS) has brought out its 18th annual multi-specialty data report that provides useful insights about the growing demand for aesthetic procedures in the country. As per the report, Americans spent over $12 billion on invasive and non-invasive cosmetic procedures in 2014.

One of the encouraging findings of the report was that aesthetic procedures are gaining popularity among men on a wider scale. In the last five years, the number of men opting for cosmetic surgery and non-surgical treatments has jumped by as much as 43%.

The topmost procedures in terms of year on year growth are buttock augmentation (up 86%), labiaplasty (up 49%), and non-surgical fat reduction (up 42%).

Most Popular Cosmetic Surgery Procedures

The most popular cosmetic surgery procedures for men and women combined in 2014 included the following:

  • Liposuction (342,494 procedures)
  • Breast Augmentation (286,694 procedures)
  • Eyelid Surgery (165,714 procedures)
  • Tummy Tuck (164,021 procedures)
  • Nose Surgery (145,909 procedures)

Liposuction can be performed both as a standalone surgery and in combination with other procedures for more effective outcomes. Improve qualities in breast implants have led to a greater popularity of breast augmentation procedure over the years. Abdominoplasty, eyelid surgery, and rhinoplasty continue to be listed among most requested procedures.

Most Popular Non-surgical Cosmetic Treatments

The most popular non-surgical cosmetic procedures for both men and women combined included the following in 2014:

  •     Botulinum Toxin (3,588,218 procedures)
  •     Hyaluronic Acid Based Fillers (1,696,621 procedures)
  •     Hair Removal (828,480 procedures)
  •     Chemical Peel (484,053 procedures)
  •     Microdermabrasion (417,034 procedures)

FDA approved Botulinum Toxin injectable treatments to reduce aging signs from the face topped the list of non-surgical procedures with nearly 3.6 million treatments in 2014. Hyaluronic fillers such as Juvederm and Restylane, and laser hair removal, chemical peel treatment, and microdermabrasion have also emerged as in-demand procedures.

Growth in Male Cosmetic Procedures

According to the ASAPS records, the number of men’s procedures since 2010 has gone up as follows:

Cosmetic Surgeries

  •     Blepharoplasty (up 34%)
  •     Male Breast Reduction (up 33%)
  •     Facelift (up 44%)

Non-Surgical Procedures

  • Botulinum Toxin (up 84%)
  • Hyaluronic Acid Based Fillers (up 94%)
  • Nonsurgical skin tightening (up 37%)
  • Intense Pulsed Light (up 44%)

The future bodes well for both surgical and non-surgical cosmetic procedures, considering the past and present market trends. Just think how sunny these numbers would be if the economy was not in a recession as well. If there more jobs for people, and there easily could be, this industry would only be looking brighter.

shutterstock_102010213Among various aesthetics procedures sought by men and women, the procedures designed to achieve a flatter and firmer abdomen are more popular. However, many patients are confused between the options of tummy tuck cosmetic surgery and liposuction.

Dr. Thomas Fiala is an eminent, board certified plastic surgeon providing both tummy tuck and liposuction procedures. During the initial consultation will discuss both treatment options and recommend the appropriate procedure as per the patient’s unique needs. Dr. Fiala receives patients from Orlando, Altamonte Springs, FL, and surrounding communities.

Removal of Excess Fat and Skin

Tummy tuck is an ideal aesthetics procedure if the patient suffers from stubborn fat deposits in the abdominal area, along with the problem of loss of skin elasticity. Some patients may have weakened underlying abdominal muscles, which can also be tightened with tummy tuck cosmetic surgery.

Liposuction, on the other hand, is an excellent procedure for removal of excess fat, but not for skin or muscle tightening. If the skin elasticity is marvelous in the tummy area, and underlying muscles are strong, the plastic surgeon may recommend liposuction to remove excess fat and restore an attractive, youthful abdomen. Patients in Orlando, Altamonte Springs, FL, and nearby areas have an opportunity to receive both procedures from Dr. Fiala.

Complexity of Surgery

Liposuction is a relatively less complex surgery because it only involves removal of unwanted fat deposits from a targeted area around the tummy. The procedure can be performed under local anesthesia when tumescent liposuction technique is used. Therefore, many patients prefer liposuction because of the efficiency of the procedure.

Tummy tuck surgery is a comparatively complex procedure, particularly when underlying muscle tightening is involved. Incision is typically longer in case of tummy tuck, and scarring will also be more than what a patient can expect in the case of liposuction. However, a skillful surgeon will try to hide the scar in the natural skin crease in the lower abdomen.

Duration and Recovery

Liposuction procedure will typically be of a shorter duration, if only fat removal from the abdominal area is involved. In most cases, the patients can return home the same day. Recovery period may be around a week, after which the patient is usually able to resume their regular activities or go back to work

In case of tummy tuck, the procedure may be of a longer duration because skin excision and tightening of muscles is also involved, apart from fat removal. General anesthesia may be required in some cases, and it may involve an overnight stay at the surgical facility, depending on the patient’s condition. Recovery in case of tummy tuck surgery can be about 10 to 14 days, and full results will establish over several months as swelling subsides completely.

Longevity of Results

Results in case of both tummy tuck and liposuction are long lasting and sustainable. However, the procedures are not a substitute for diet and exercise, and the patient needs to maintain a regular exercise regimen and follow a balanced diet. Patients should be made aware during the consultation that both liposuction and tummy tuck are not weight loss procedures.

Getting a incision to look fantastic after surgery is a combination of several factors:
   a)  proper placement of the incision
   b)  tidy suture closure, done in multiple layers
   c)  avoidance of tension
   d)  … and a little bit of luck.
Recently, we’ve been using a new product at our office – and I wanted to share the early results with you.  It’s a special dressing, called “Embrace”, made by a start-up company called Neodyne.  Two plastic surgeons, Michael Longacre and Geoff Gurtner, are the brains behind the outfit.  (I know Geoff from surgical residency days.)
The dressing is a stretchy silicone material that is placed across the incision in the stretched position, then attached.  As the dressing retracts, it reduces the tension on the incision.  And when worn for 8 weeks after surgery, this off-loading of the tension not only causes less widening of the scar, it results in less thickening and a better cosmetic appearance.  This has been proven in several studies.
We’ve been recommending this dressing on all of our recent tummy tucks and body lift procedures.  And the early results have been impressive – it really works.  The people who use this product are seeing significant cosmetic improvements.
Here are some photos from the company – these are very similar to the results we are seeing.  The untreated scar is on the left, and the treated scar – from the same patient – is on the right.
The American Society for Aesthetic Plastic Surgery has released its report on plastic surgery in the USA for 2013, based on recent surveys of plastic surgeons, otolaryngologists and dermatologists.
Here are some of the highlights of the report:
  • Over 11 million surgical and non-surgical cosmetic procedures were performed in the US in 2013.   Over 85% of these were non-surgical, such as Botox, fillers or laser treatments.
  • Compared to 2012, there was a 6.5% increase in the total number of cosmetic surgical procedures.
  • The most popular surgeries were:  (#1)  liposuction and (#2) breast augmentation.  These two procedures reversed their order from last year’s report.
  • The most popular non-surgical procedure is: Botox, with over 3.7 million injections performed last year.
  • Buttock augmentation is on the rise, with 11,500 procedures being done this year, compared to only 7,200 the year before.  This procedure wasn’t even on the radar screen just a few years ago.
  • Women accounted for 90% of all surgeries.
  • The most common procedures for men are: (#1) liposuction, (#2) cosmetic eyelid surgery.
More details will be published shortly at the ASAPS website (
With more emphasis in pop culture on the shape of the derriere, more patients are coming to see us about the idea of buttock augmentation.  One of the most common questions I hear is:  Which is better – the brazilian butt lift or buttock implants?
While both methods can work, for patients that have a reasonable amount of fatty tissue that we can borrow, the brazilian butt lift – also known as fat transfer to the buttock –  clearly wins.  Here’s why:
  • you get the twin benefits of buttock enhancement and the slimming effect of liposuction of the waist, abdomen, thighs or wherever the fat is taken from
  • fat grafting, when performed by experienced plastic surgeons, is a safe procedure and has a lower complication rate than buttock enhancement with solid silicone implants
  • contouring of the buttock is much more flexible with fat grafting.  When a buttock implant is used, there are a limited number of sizes and shapes.  You are also limited about what size of implant can fit in an intra-muscular pocket.
  • Post-op recovery is more painful with buttock implants.
  • Some complications, like migration of the implant, capsular contracture, or wound separation – can only happen when solid buttock implants are used.  They can not happen with the fat injection technique, as the surgical method doesn’t involve making an incision and a large pocket.  Since fat transfer uses your own tissue, there is no risk for capsular contracture.
That being said, there is one group of patients who do better with buttock implants:
  • patients that don’t have enough fat to graft, and can’t or won’t gain weight
Additionally, fat grafting to the buttock does have a downside.  Sometimes that fat that is transferred may not survive the move, and may melt away.  A second fat transfer procedure may be required to optimize the size and shape of the buttock.. But that is usually much easier than the revision of a buttock implant.

How often do you hear about “no recovery” or “lunchtime” treatments when reading about lasers, peels and other treatments in the media?  Until recently, there was no accepted definition for these terms. What does “minimal recovery” really mean – minimal to whom, the doctor, or to you?

The American Society for Aesthetic Plastic Surgery (ASAPS) has developed a set of standardized definitions that consumers and industry can use, when talking about issues like downtime, bruising, redness, swelling and pain.  We’re trying to encourage the cosmetic laser industry to adopt and use these terms in a consistent and honest way, so that consumers can really fully understand what they’re choosing, and can compare treatments while shopping around.

Downtime –the expected time to return to normal lifestyle

Essentially None: less than 24 hours

Minimal: 24-72 hours

Moderate: 3-7 days

Significant: more than 7 days

Bruising – visible on the skin without concealer

Essentially None: no bruising but there may be an immediate change in skin tone

Minimal: less than one week

Moderate: 1-2 weeks

Significant: more than 2 weeks

Redness –visible without concealer

Essentially None: returns to normal (pretreatment or improved) in less than 24 hours

Minimal: 1-3 days

Moderate: 4-7 days

Significant: more than 7 days

Swelling –obvious swelling

Essentially None: less than 3 days

Minimal: 3-7 days

Moderate: 8-14 days

Significant: > 14 days

Pain –significant discomfort associated with the treatment

Essentially None: no anesthesia or medication is needed except over the                                      counter  medication

Minimal: requires pretreatment with oral prescription medication, topical anesthetic agents                or skin cooling and/or post-treatment prescriptions for pain management.

Moderate: same as minimal but with local anesthesia (injections)

Significant: same as minimal but with IV sedation or general anesthesia

On Friday, Sientra announced a new warranty program, which they are calling “C3”.  The program, which begins Novemeber 1, 2013, will apply to first-time breast augmentation patients who choose Sientra-brand textured implants.
In addition to the current implant deflation warranty, the C3 program provides a free replacement implant if the patient requires revisional breast surgery for a Baker Grade III or IV capsular contracture, within the first two years after the original implantation surgery.
No additional registration forms or fees will be required.
Capsular contracture is one of the most frustrating and unpredictable complications after breast implant surgery.  Previous studies have found that it happens more often in the subglandular implant, if a periareolar or trans-axillary approach is used, or if there is bleeding or infection in the breast.  In some studies, smokers have an increased risk.
In the upcoming November issue of Plastic & Reconstructive Surgery, a new study looking at Sientra implants also found a protective effect for their textured breast implants, when compared to the smooth Sientra implants.
Bottom line:  I’m sure that patients who choose Sientra’s gummy bear implants will be pleased to hear about this program, going forward.  It will be interesting to see how the other established manufacturers – Allergan & Mentor – react to this new program.
